Hospice care is a patient and family-centered approach to medical care and support for people who are near the end of life. The hospice care team includes, a doctors, nurses, social workers, counselors, and trained volunteers. In addition, family is considered an essential part of hospice care, and we encourage loved ones to be involved.
The interdisciplinary Hospice of New Mexico team consists of:
- The patient and patient’s family
- The patient's personal physician
- Hospice medical director
- RN case managers
- Home health aides
- Licensed social workers
- Counselors
- Therapy and treatment specialists
- Hospice-trained volunteers
The hospice team works together focusing on the dying patient’s needs; physical, psychological, or spiritual. The goal is to help keep the patient as pain-free as possible, with loved ones nearby until death. The hospice team develops a care plan that meets each patient's individual needs for pain management and symptom control.
